Is Orthomolecular Treatment a "Snake Oil"?

       In a Wikipedia entry about orthomolecular treatment a pediatrician and author named Julian Haber described the "orthomolecular" approach as "snake oil." I was confused how this statement could be so flippantly asserted. I pondered the substance of this assertion. The I investigated the author of the "snake oil quote" and the person who invented the term "orthomolecular treatment" to compare expertise and perspective.

Pondering the content of the Snake Oil Claim  
       "Snake Oil" alludes to the sale of tonic or organic oil peddled as cure for every ailment described to the gullible, naive  and desperate.  It brings up visions of salesman traveling through the rugged west of the United States stopping at small towns to sell bottles of the liquid to cure baldness, skin rashes, fever, headaches, fungus, swelling, headaches and whatever else might be ailing settlers desperate for any medical supplies.  After the pitch and sales the salesman would pack up his wagon and leave.  He would depart before data was gathered that might discount the claims. and move on to "new markets".  The salesman made money even as the "oil" failed to "cure".  Given the poor state of communication at the time there was little to fear in moving on to the next community.  There was no systematic evaluation of the outcomes available.  Ignorance, lack of data and mystery helped keep the markets available to the salesman here today who was then gone tomorrow.  The ruse was never exposed.
       The "snake oil" analogy appears at odds with orthomolecular approach.  Today orthomolecular approaches are being implemented world wide (though most popular in Australia).  Those using orthomolecular approaches use client interviews and analysis of blood, urine and hair.  This analysis leads to a nutritional program that may meet the needs of a particular individual.  There is no one size fits all vitamin, tonic, pill or powder as touted by the "snake oil salesman".   The Orthomolecular approach uses a variety of nutritional supplements from a variety of sources.  The snake oil salesman sought to avoid discussion of specific mechanisms, study, research or evaluation.  The Orthomolecular approach now has a rich research base with data gleaned from advanced techniques like resonance scanning, radioactive tracers, post facto studies and clinical trials among other techniques.  The data  are published in peer reviewed journals accessible to both the medical community and to the public.  In one example the The Journal of Orthomolecular Medicine has been publishing for over 3o years.  Significant studies in other similar journals are increasingly being cited in medical journals.  Orthomolecular practitioners like  Dr. Walsh, Dr. Mensah, Dr. Selig and many others collect data, publish and train doctors.  These leaders and leading journals do not change names and move on.
      So the suggested parallels between the "snake oil salesman" and the orthomolecular practitioners appears to be lacking substance.  "Name calling" fails to illuminate the differences for those who are ignorant.

How do the Authors Compare in Expertise?
      This quote might have one classify the leaders of the orthomolecular treatment movement as untrained and ignorant with little or no credentials selling "snake oil" to the gullible.  Dr. Haber who disparages the Orthomolecular approach is depicted as credentialed and "highly informed".   Dr. Haber had numerous articles and poems published in Good Housekeeping, American Veteran, Critique Magazine, Fort worth Star Telegram, Healthy Kids Magazine, American Family Physician, Di-Verse-City Anthology and Blood and Thunder.   Dr. Haber writes both fiction and non-fiction so he is very articulate (sort of like a snakle oil salesman?)  One wonders how would Dr. Haber compare with the person who coined the term "orthomolecular"?
     The person who coined the term "orthomolecular"  was Dr. Linus Pauling (1901-1994).  In 1954 Dr. Pauling was awarded an unshared Nobel Prize in Chemistry.  He is the undisputed founder of quantum chemistry , molecular biology and was considered a world renowned expert in biological molecules while he was alive.  He  published  over 850 peer reviewed scientific articles and books and 350 other articles and books (but none of them fiction).  Dr. Pauling obtained a B.S. from Oregon State University and his Ph.D. from the California Institute of Technology.   Dr. Pauling  was named one of the 20 greatest scientists of all time and was featured as a scientist "stubbornly ahead of his time" by those who endorsed him in the prominent publication "Scientific American" (1993).
      During the pinnacle of his professional career at age 40 Dr. Pauling was diagnosed with Bright's disease.   This was a serious kidney disease (now called glomerulonephritis) that was fatal with no treatment for renal failure available until  20 years later.  Linus Pauling worked with a nutritionist Thomas Addis, and Ava Helen Pauling as "nutritionist, cook, and eventually as deputy 'doctor'" to manage the disease with a low-protein salt-free diet and vitamin supplements.   Dr. Linus Pauling lived, worked, researched and published another 53 years.  His first success using the orthomolecular approach was very personal.  What one author calls "snake oil" had saved his life.   
     In 1965, Pauling read Niacin Therapy in Psychiatry by Abram Hoffer and theorized vitamins might have important biochemical effects unrelated to their prevention of associated deficiency diseases.  Pauling coined the term "orthomolecular" to refer to the practice of varying the concentration of substances normally present in the body to prevent and treat disease.  In 1968, Pauling published a brief paper in Science entitled "Orthomolecular psychiatry", giving a name to the popular but controversial therapy.  Dr. Pauling speculated that "orthomolecular therapy, the provision for the individual person of the optimum concentrations of important normal constituents of the brain, may be the preferred treatment for many mentally ill patients."   His ideas are not generally practiced by conventional medical professionals, has been strongly criticized BUT since 2005 this approach has spawned increasing numbers of new research in mainstream medical publications with many found in the US National Library of Medicine National Institutes of Health (with multiple peer reviewed journals).  
      So an author of numerous fiction and non-fiction works is quoted in a manner that depicts a Nobel Science Winner with 1200 non-fiction publications  as a snake oil salesman?

Does Science Change?
     Changes in science disciplines rarely come easy or quickly.  In one example of change it took 70 years and numerous false sightings of a hypothetical planet "Vulcan" to confirm that the planet  did not exist and that General Relativity was more correct than Newtonian gravity.  In another more recent example the geologists who claimed tectonic plates moved entire continents were mocked and deemed incompetent for three decades by fellow professionals until "continental drift" became widely accepted.  In Psychiatry prior to the 1970's mothers of Autistic children were deemed "frigid" and expected to submit children for endless "therapy sessions" for what is now known to be a biological pathology.  In The Broken Brain by Nancy Andreason (MD, PhD, Psychiatric Chair and winner of numerous awards for professional courage) was met with similar skepticism almost 40 years ago and now mainstream psychiatry recognizes that biological pathologies underlie all mental illness.  Times are still changing.  In the same way it appears that Dr. Linus Pauling may be  proving to be "stubbornly ahead of his time"  when it comes to "orthomolecular treatment."
     Mainstream medical research is only recently beginning to recognize the impact of nutrients.  They are years behind but now beginning to see that statistical truth.  Listed below are just a limited sample of recent studies listed at  US National Library of Medicine National Institutes of Health:

          A 2018 study finding links with mental health links to Vitamin C:
     Linus Pauling forfeited incredible income as a spokesman for pharmaceuticals when he took up the orthomolecular cause.  The Wikipedia article may reflect the perspective of pharmaceuticals as sales and public relations persons in industry often edit the content of the articles in Wikipedia.   It is no secret that pharmaceutical  interests  seek to discourage orthomolecular approaches (as orthomolecular approaches offer cures rather than maintenance.)  There may pediatricians like Dr. Julian Haber who may prescribe medications than manage the illness.  The financial model for doctors and pharmaceuticals benefit greatly from long term dependence on medications.    Is it possible that curing a metabolic dysfunction with nutrients aimed at the a cure is not a sustainable economic model?
   

      Below are a sample of studies in the US National Institute of Health ; in other national peer reviewed journals appear to agree with Dr. Linus Pauling:

"with the B vitamin constituents. These conclusions received further support from a more recent study that also demonstrated improved mood following four weeks of supplementation with a multivitamin containing high levels of B vitamins."   
  
"[vitamin] B showed significant improvement on the mental health scale of the SF-36 compared to placebo. "

 "On the basis of current data, we suggest that oral doses of both folic acid (800 microg daily) and vitamin B12 (1 mg daily) should be tried to improve treatment outcome in depression."    

"evidence links low status of folate and the related B-vitamins (and/or elevated concentrations of homocysteine) with a higher risk of degenerative diseases including cardiovascular disease (CVD), cognitive dysfunction and osteoporosis." 

"Current research in psychoneuroimmunology and brain biochemistry indicates the possibility of communication pathways that can provide a clearer understanding of the association between nutritional intake, central nervous system, and immune function thereby influencing an individual's psychological health status. These findings may lead to greater acceptance of the therapeutic value of dietary intervention among health practitioners and health care providers addressing depression and other psychological disorders."

After examining a variety of studies that detail the impact of B and numerous other nutrients this review of studies states:  "As researchers continue to investigate the efficacy of nutritional healing, more and more information will be readily available to those wishing to take a more natural course of therapy. For the time being, we can rely on the already thousands of successful studies that “alternative therapies” need not be an alternative anymore."

THE BROKEN BRAIN
The Biological Revolution in Psychiatry

     One might wonder why a site dedicated to Pseudomemories would review and examine a book about mental illness.  The reason is because those who are mentally ill (or close to it) are more vulnerable to the techniques often used by memory based therapists to illicit false memories of abuse (often the counselor unknowingly creates such memories).  In fact exposure to such horrid and emotionally jolting ideas and images can initiate physiological response involving hormones and dysfunctional metabolic cycles that lead to delusions strengthening false suspicions, paranoia that inoculates the mentally ill person from questioning the delusions and may even lead to psychosis with hallucinations.  We now know this from personal experience.  
    A mentally ill person began accusing my husband.  The police were unable to tell mental illness was involved, investigators were not sure if the bizarre horrid accusations were true or not, counselors said they could not help.  After we were  able to get  appropriate treatment (medication to correct the dysfunctional metabolism) the mentally ill person recovered, was able to distinguish hallucinations from reality and then able to dispel the horrid delusions that had been suggested but not true.  When a person begins to make unfounded and false accusations based on psuedomemories it is strongly advised that the person(s) accused immediately focus attention on the psychiatric condition of the accuser.  It not only is a good defensive move it also may help the accuser get needed treatment.
     We were not aware of the principle of caring for your accuser's mental health when first confronted with accusations and only later discovered how important it is.  We were accused by a person in 2015 and were totally unprepared and ill advised.  When a second person accused us four years later we immediately, based on our more informed awareness,  focused on this person's mental health.  The second person is now doing very well after getting appropriate treatment.  The first person continues to struggle, clinging to delusions, experiences paranoia, spreads horrid images to children in her care, seeks funding using slander, remains estranged from all members of her nuclear family and displays emotional instability (eruptions of rage reported to us by people still interacting with her) and insists on "being cut off" from those who do not share and support her delusions. 

      This is a classic book that lays down the case for understanding the true nature of mental illness in a new way.  It was published in an America where psychoanalysis and social determinism dominated psychiatry for over a century.  Dr. Andreasen was a revolutionary when she wrote this book.  She used her impeccable credentials, displayed courage in taking a stand and evidenced competence in making the case presented in the text. It has been over three decades since she wrote the book and her original perception has transitioned from revolutionary to increasingly mainstream.  Dr. Andreasen saw what now seems obvious now but at the time was only a murky idea.  She asserts the now widely accepted idea in psychiatry that curing all mental illness requires biological solutions.
     The book begins with a brief historical perspective on mental illness that can be gleaned from ancient times to recent centuries.  Ancient writings that depict organs of the body shifting and moving as an explanation.  Demons and spirits taking control of the body from the person stricken with mental illness.  Finally a belief in the depravity of soul and spiritual darkness morphs over centuries into the idea that parents and social interactions nurture a person for a lifetime of mental suffering.  Numerous historical understandings of mental illness are detailed, documented and evidenced over the first four chapters.
     The next three chapters detail the mechanisms that underlie how the brain works and how it interacts with the body that maintains it.   The effort to explain the science of the brain is ambitious as she details anatomy, chemistry, cellular actions in the neurons and hormonal interactions with the body. The information is accurate at the time the book was written and it is now supplemented with knowledge of metabolic pathways that build the proteins and neurotransmitters used behind the blood brain barrier.
I particularly like the detailed analysis of the twin studies. Dr. Andreasen does an excellent job explaining how twins separated at birth and placed in a variety of homes provided an understanding that genetic predisposition fully explained the observed rates of mental illness. Social environment was found to be a minor contributor (if it existed at all) to the appearance of mental illness. Twins studies (a number of them) showed mental illness rates unrelated to family assignment and that genetic modeling alone predicted the rates of mental illness emergence.
The book explains with a quick description of how proper diagnosis and appropriate medication yield what seem to be miraculous results. These results of medications are fully explained and detailed as biological solutions for mental illness that are verified to work on patients who suffer from the symptoms.
The closing of the book addresses something our society has still not resolved. A person with cancer or some other somatic illness is assisted, supported, encouraged and often given reduced workloads to assist in overcoming the illness. The mentally ill person (and family members) today are often shunned, isolated, estranged from family, may forfeit a professional licence, lose a career or be terminated do due to a mental illness. The result is that the mentally ill seek to avoid diagnosis and treatment likely prolonging and exasperating the illness. The author does not think this a humane way to treat a person who is suffering from any biological pathology including those who have a mental illness.
This book was far ahead if its time in the 1980's but our biological understanding has advanced significantly since then. I would love to see another book like this that includes the explosion of knowledge about metabolic synthesis of neurotransmitters behind the blood brain barrier (orthomolecular studies included), the likely autoimmune dysfunctions that underlie the emergence of the pathologies and the possible impact of our understanding of epi-genetics. All these areas have increasingly emerged as sources of biological pathologies underlying mental illness since 2005.

This book is aimed to transition psychiatry (and society) to a new way of thinking about mental illness. Newtonian dynamics was used to discover new planets but also led to the false belief in a planet called "Vulcan" between Mercury and the Sun (explaining orbital variations of Mercury). This widely held belief in a planet that did not exist was "settled science" from 1840-1910. Later the orbit of Mercury was found to be fully explained by a new theory for Gravity called General Relativity. In another example geologists began to see that moving continents over eons of time were responsible for geological features and seismic activities on the Earth today. "Plate tectonics" was openly ridiculed from 1940-1960 (in peer reviewed Geological Journals) but is now a widely accepted principal. A predominance of psychiatrists once thought that parental approaches or traumatic childhood experiences were at the root of mental illness. Beginning in the the 1950's psychiatrists increasingly came to see mental disorder as a direct result of biological pathologies. Major transitions in thinking have happened in other disciplines but this revolution is very different because how our culture views the interaction of freedom and responsibility. The ideas that moral weakness, dysfunctional parenting and/or childhood experiences are the root cause of mental illness will be difficult to overcome among the less informed. The association of mental illness with moral failings and/or inadequate parenting still hinders data collection even today. Families often scramble to protect themselves from darkened reputations, lost careers or even prison for real or imagined "moral failings" by hiding and obscuring the episodes brought on by mental illness. This book is confident that society will increasingly becoming open to the data and facts and that the truth will emerge (even if slowly). This book is helping shorten the transition to a more humane and truthful society.

Health and Pseudomemories


Physical Health and Pseudomemories

      Those who adamantly believe that pseudomemories are factually accurate memories may be suffering from mental illness and may need medical care.  This elephant in the room is rarely discussed  A person who is physically able to walk, listen and talk in a manner that appears coherent  is assumed to be in good health.  Even when a person appears to be in good health the fact is that the physical dysfunctions that underlie mental illness can create lucid delusions and vivid hallucinations.  Delusions and hallucinations which seem real to the mentally ill can and do create vivid  memories of events that have not actually occurred. 
      In the 1960's and 70's studies of twins separated at birth revealed the the emergence of mental illness occurred at the same rate for those twins raised together or separately in different families.  The long held assumptions that parental approaches, personal trauma and life conditions created mental illness like schizophrenia, depression and bipolar disorder were questioned.  Additional research revealed that the nurturing conditions and personal life experiences alone did not explain the emergence of mental illness.
      Dr. Nancy C Andreasen MD, PhD and chair of the Psychiatric Department at the University of Iowa has done extensive research on mental health using brain scans,  written psychiatric textbooks and popular texts on mental health and received numerous awards over decades of service.  In the "The Broken Brain: The Biological Revolution in Psychiatry" (1985) she detailed and proposed the now widely accepted view that biological processes explain mental illness.  In receiving a presidential award for science in 2000 it was stated:
     "Long before it was broadly acknowledged, Nancy Andreasen understood that mental 
     illnesses are biologically-based brain disorders,  Her groundbreaking research on 
     schizophrenia has inspired hope for improved treatment and recovery..."
     A variety of physical dysfunctions are now known to create a schizophrenia and the psychosis that comes with this illness including metal toxicity,  food allergies, undermethylation, over methylation and pyrrole disorder.  Recreational drugs, medication, sleep deprivation  and even excessive water intake can create psychosis that results in hallucinations and easily created delusions.
     A well known Psychiatrist, Dr. Abram Hoffer, wrote a book on the treatment of those with schizophrenia and shared his insights regarding how to help the patient emerge from the illness in a book titled "The Orthomolecular Treatment of Schizophrenia" and on page 40 of that text he states:      
     "...I find it very helpful to have the family in my office during the interview, with the
     patient's permission. A lot of time is saved in gathering the information needed to make
     the diagnosis.  It also helps when the family hears the discussion and knows what the 
     treatment will be.  Although most psychiatrists no longer blame the family for the illness
     of a relative, it can still occur.  It is important to remove any guilt them family may have
      derived from previous outmoded, harmful explanations."
Dr. Hoffer recognized, in the quote above that therapists with good intentions could implement errant treatments, like memory based therapies,  that create estrangement of the family from the patient.  He was aware that successful treatment of those suffering from schizophrenia often relied on families and family members that may have been part of horrid pseudomemories generated by the patient while in psychosis.

    Dysfunctional metabolisms create psychosis and false memories may emerge from the resulting hallucinations and delusions. These false memories can be believed by the afflicted person as real. False memories can then be inappropriately used in memory based therapies, in family confrontations, in legal actions, result in loss of careers and even imprisonment of accused innocents. The innocent but accused person should be advised to focus concern on the possibly undiagnosed mental illness of the accuser.

Review: OTHOMOLECULAR TREATMENT FOR SCHIZOPHRENIA


Orthomolecular Treatment for Schizophrenia
by Dr. A, Hoffer, M.D., Ph.D.

.....This is a very brief book (50 pages of prose) focused on providing the reader with illustrations and examples of how orthomolecular treatment helped his clients who often suffered horribly from the ravages of schizophrenia.
.....While his selections of case histories is varied and illustrative of the various forms and presentations of Schizophrenia this is not a text useful for formulating a specific treatment.  It is written to help those facing this illness realize that there are other options for those afflicted with Schizophrenia besides a lifetime of reliance on medication aimed at resolving symptoms.  This medication often has permanent side effects that are known and futre side effects that have not been foreseen.
.....The book is well documented with 76 research studies listed and referenced wherever Dr. Hoffer makes a factual assertion.  So while the short text is easy to read, avoids explaining probable mechanisms and bogging the non-technical reader down it also provides links to well done research often in reputable publications.
.....The book was of interest to me because my daughter suffered from horrid, uninformed even if well intended treatment that delayed her care.  Dr. Hoffer writes about the mentally ill due to metabolism dysfunction and nutritional imbalances coming to him from families who had initially gone to professionals using debunked "recovered memory techniques" in the 1980's and 90's when the technique was popular.    Dr. Hoffer understood the importance of intact and caring families to support and help the mentally ill recover.  He commented on this experience: "...I find it very helpful to have the family in my office during the interview, with the patient's permission. A lot of time is saved in gathering the information needed to make the diagnosis.  It also helps when the family hears the discussion and knows what the treatment will be.  Although most psychiatrists no longer blame the family for the illness of a relative, it can still occur.  It is important to remove any guilt them family may have derived from previous outmoded, harmful explanations."
.....I give the book 5 stars because it has a focus on case histories and research substantiated positions that can help anyone who is ravaged by mental illness or has a family ravaged by mental illness.  The book was written over 20 years ago now and much more about the physical pathways to schizophrenia is known now that was not known then but the details of those pathways are not important to the impact of this book. Despite the impediments to the widespread use of orthomolecular techniques during his lifetime Dr. Hoffer saw the change coming and he had great hope:
"..If and when orthomolecular therapy is widely used and is widely accepted, the whole field of the biochemistry of schizophrenia and its treatment will flourish.
...It would be very exciting to come back in the year of 2020 to see what happened to this field."
I do hope he is right!

Psychiatrist Dr Abram Hoffer MD PhD treated and cured patients with Schizophrenia until his death in 2009, for fifty five years with Vitamin B3, otherwise known as Niacin. This publication points out that Pellagra and Schizophrenia are one and the same disease, one being more intense than the other in its requirement of nutrient supplementation.

Anyone who has a psychiatric disorder should be encouraged to look into Vitamin B3 (Niacin) to get off psycho-pharmaceutical drugs that don't offer a cure.

My son was diagnosed with schizophrenia at age 18. He was hospitalized and released. He was taking resperidral and it stopped working. Doctors put him on another med which didn't work. I started doing some Internet research on alternative choices and came across orthomolecular medicine for schizophrenia. We read the books and contacted an orthomolecular psychiatrist in our area and my son started on the supplements right away. He felt better within a day. He returned to his original medication as well and he is feeling better than ever. His personality is back his symptoms are minimal. He's going to school and working. He's more organized. Hes thinking and planning his future. He's laughing and engaged with others. This has been a life changing experience for him and everyone in our family. I urge everyone who has the diagnosis to read the books and contact an orthomolecular doctor.

In the late 1900's psychoanalytic approaches to mental health included what came to be known as "Recovered Memory Therapy"  This therapy aimed to revive dormant or "repressed memories" (which have since been debunked by researchers and experts in memory like Dr. Elizabeth Loftus).   Horrid memories thought to be from childhood were very often pseudomemories confabulated from media depictions, horror films or subtly suggested by the therapist seeking to help the client "explain" and "understand" the impaired judgments of often mentally ill patients.  Professionals were trained in and used these "memory recovery techniques".   Many later came to reject the approach as harmful (Dr. Paul Simpson wrote a book, "Second Thoughts" about the experience.)  These "memory recovery" techniques disrupted lives, caused family estrangement and often deepened the mental maladies of clients while also increasing the possibility of  suicide.   Dr. Hoffer likely saw the mentally ill due to metabolism dysfunction and nutritional imbalances coming to him from families who had initially gone to professionals using these now debunked "recovered memory techniques" in the 1980's and 90's when the technique was popular.    Dr. Hoffer understood the importance of intact and caring families to support and help the mentally ill recover.  He commented on this experience:
